A focused clinical assessment for individuals seeking work leave, disability, workplace accommodations or related clinical documentation.
The assessment includes review of your current symptoms, relevant history, treatment needs, day-to-day functioning, work-related impairment, safety concerns and diagnostic considerations. It also allows me to determine whether the documentation you are requesting is clinically appropriate and supportable.
The $150 fee covers the 60-minute assessment and completion of the clinical assessment record. Employer forms, leave or disability paperwork, letters, record review and other requested documentation are separate because the amount of professional time involved varies considerably.
This assessment may be appropriate for:
• Work leave and return-to-work needs
• Workplace accommodations or modifications
• FMLA and disability-related requests
• Clinically supported letters based on medical necessity
